Save Time Download Registration Forms!

After you make your appointment you can download, print, read, fill out, and sign your registration forms before you arrive. Then bring them with you to your appointment. This will save you some time when you check in to the office.If you are a patient of Dr. Klarquist and it is your first time in his new office you must fill out a registration form and bring it with your so you can register at his new office.

Welcome our new physician Tom Klarquist

Tom Klarquist now is a member of Advocate Medical Group Millennium Park. Dr Klarquist has moved his practice "upstairs" to our office.We welcome Dr. Klarquist and his patients.

New Hyde Park Office Saturday Hours

Dr. Carlson and Dr. Litoff are now each seeing patients one Saturday a month at the Advocate Medical Group Hyde Park office. Appointments can be made through our office via phone or email. The office address is 1301 E 47th Street next to Bally's Total Fitness. Parking is free.

We renamed our practice Advocate Medical Group Millennum Park to showcase our location and our affiliation with Advocate Medical Group. Our office address and phone remain the same.

Valet Parking Now Available

Cash only valet parking is available in front of our building. Park for up to 3 hours for $12. This is substantially lower than any of the nearby garages and you also get the convenience of valet parking right in front of our office.
